Want to make a change in your life? Maybe you've wanted to bring your grades up, but aren't sure how. Or maybe you've been meaning to get more active, but just can't seem to make it happen. If there's an area in your life you'd like to improve, then a Wellness Plan may be for you!

Wellness Plans are a way for you to look at things in your life that are going really well and come up with a plan to help things you want to get better. To get started, follow these easy steps:

1. Sign up at the RYSE front desk or with any health/wellness counselor. You can choose to meet with Aran, Che, LaShonda, Morgan, or Sheerene.

2. Meet with a counselor to discuss what's going on in your life, both the ups and downs. Anything you say about sex, drugs, and feelings is CONFIDENTIAL unless you give permission to share it.

3. Rate the areas of your life on a worksheet from 1-5, with 5 being the happiest. Choose an area you'd like to improve. Want better grades? Happier relationships? A healthier lifestyle?

4. With the help of a counselor, make a plan to accomplish your goal. It should be something you can stick to or else it won't do you much good.

5. As soon as you leave the office, start following the plan!

6. Meet with your counselor six months later to see how you're doing. If you accomplished your goal, great! Maybe you want to tackle something new. If you didn't accomplish your goal, the two of you can think of other ways to get where you want to be.

Helpful Hint: If you meet with a counselor to make a Wellness Plan, you are entered into a raffle for a $15 Target gift card!

Your goals may seem hard to reach, but with a little help and advice, they're much easier to accomplish. So start thinking about what YOU want to achieve and sign up for an appointment today!
